We prefer applicants to submit answers to the questions from the Lead Project Coordinator and the other staff person or organization leader involved in the project. If they don’t yet have the other staff person identified, they should have an organization leader respond. Final decisions about project staff participating in cohort will be determined when awards are made.
The reviewers will be given your application from Step One with the additional information requested in this Step two of the application process.
As part of our new strategic plan, the Center for Prevention has identified 4 strategies to achieve Health POWER initiative goal of building community capacity:
For more details on these strategies, see the PDF called “Our Approach.”
The second tab, “instructions”, in this budget form should give you more guidance. Indirect and fringe benefits should not be a separate line item, but should be incorporated into the consultant’s hourly rate.
Please click on the “Attachment” tab and upload the document with the answer to your questions under “Letters of Commitment and Support.” Please see the screenshot below.
Please note, the system will not allow you to upload word documents – you must convert your documents to PDF before uploading them.
Awardees for the Health POWER initiative will be announced on October 1, 2019.
The application process:
STEP 1: All applicants will first need to submit a proposal following the requirements in this RFP.
STEP 2: Based on an internal review of the submitted proposals, selected applicants will be invited to submit additional information that may include, but not be limited to, more detail on their proposed work, budget for the proposed work and staffing to deliver the services. Blue Cross will conduct site visits of applicant finalists before making final funding decisions and negotiating contracts with successful applicants. All contracts are subject to successful contract negotiations between the applicant and Blue Cross.
We will be contacting finalists for site visits on Sept 6, working to set-up visits the weeks of September 9 – 20, 2019.
